Antidote:
While the antidote flumazenil can effectively reverse benzodiazepine-mediated sedation, its use in acute benzodiazepine intoxication is not recommended due to the potential risks that may outweigh benefits as it can precipitate acute withdrawal syndromes, lower seizure threshold, and cause cardiac dysrhythmias.
Pharmacokinetics and Dynamics
Metabolism:
Metizolam is metabolized in the liver via the cytochrome P450 enzyme system, primarily CYP3A4, similar to other benzodiazepines and thienodiazepines. The resulting metabolites are excreted primarily in urine.
Drug-Drug Interactions:
Metizolam has additive effects when combined with other central nervous system depressants, including alcohol, opioids, barbiturates, and sedative antihistamines, significantly increasing the risk of respiratory depression, sedation, and overdose. It may also interact with drugs that inhibit or induce the CYP3A4 enzyme, potentially altering its metabolism and potency.
